One of the most embarassing moments in video game history was when the IGN watermark ended up being on the Okami Wii game cover. A close second however comes from the newest Guitar Hero game, Guitar Hero: Metallica.
The cover features the Guitar Hero: Metallica logo, with all of the bands featured in the game written behind it. Unfortunately, Rolling Stone had realized that the band Lynyrd Skynyrd was incorrectly spelled as Lynyrd Skynrd.
Of course the band's name isn't that easy to spell in the first place, so we can't rag on Activision for missing such a thing. In fact, maybe we should blame the band, for not just going with something like Leonard Skinnered, a name which only really suits an 87-year-old Jewish rabbi anyway, but I digress.
